This survey is a part of the research project called “Use of a Supportive Care Screening tool in Blood Cancer patients undergoing treatment; feasibility and success of an electronic PROMs (Patient Reported Outcomes Measures) and nurse led assessment to meet patients’ wellness needs: a pilot study.” It is called the ePROMs study for short.
This research project is looking at screening for supportive care needs in blood cancer patients, who are receiving treatment at the Cabrini Haematology and Oncology Centre.
This survey is only to be completed by patients who have signed a consent form to participate in the study. It is designed to be completed at every visit to the outpatient centre, or prior to Telehealth appointments. Participants are also welcome to complete the survey at home as needed. Please be mindful that survey responses will be viewed and actioned within 3 days. For emergencies, please call our 24-hour contact number for the on-duty haematologist (0499 300 777) or call 000.
